Federal Employer Identification Number (FEIN) Form Instructions

  1. In line 1, make sure you enter your legal name (as it appears on your Articles of Incorporation, if you have one).   
  2. Once the form is filled out, make one copy for your records and one copy for the APRA office. There is no fee for this.
  3. Send the original form to the Internal Revenue Service (IRS) either by mail or fax. You must send the form to the IRS Center that corresponds with your state. See the form instructions on “Where To Apply” to find your IRS Center’s address. It takes about 4-5 weeks to receive your Federal Employer Identification Number (FEIN). Note: Do not mail the application if you faxed it. The IRS will interpret this as a request for a second FEIN.
  4. When you receive the notice of your chapter’s FEIN from the IRS, send a copy of it to the APRA office.
